This module aims to address the growing prevalence and complexity of managing chronic conditions in the community with an increasing need for long term management.
By completing this module, the student will be empowered to provide comprehensive assessment and person-centred care for individuals with chronic and palliative conditions. The module will allow the student to possess the necessary knowledge and skills to manage the complexities of chronic disease and make informed decisions to support patients in their long-term management to improve health outcomes and overall quality of life.
Understanding person-centred care for chronic conditions, comprehensive assessments and decision making will be enhanced through a critical understanding of evidence-based practice.
